This defines High Reliability Organizations: the aircraft carriers, air traffic control centers, and nuclear facilities that have remarkably few catastrophic failures. The DOE's framework treats human performance improvement as one of the main routes to being recognized as a High Reliability Organization.

What does human performance have to do with engineering?
Here's the part that matters for engineering firms, and it's the point most discussions of human performance skip entirely.
Those latent conditions (hidden weaknesses) that human performance work is designed to root out don't appear out of nowhere. Many of them can be traced back to engineering and design decisions. This could be:
